SAGE CANYON WEST, LLC Guide and Outfitters Company

Release of Liability

I _______________________________ (the person signing below), will be a hunter or a guest of a hunter on the Nucor West Cooperative Wildlife Management Units (CWMU) in Utah. I will be allowed access by the Sage Canyon West, LLC Guide and Outfitters Company (Guide). I recognize that there are certain risks inherent in hunting and associated activities including but not limited to, to the following: falling down, being struck by any object, being bitten or attacked by any animal, injury due to weather, injuries while driving, or any other risks associated with hunting in mountainous terrain.

I hereby acknowledge that I agree to protect, indemnify, and hold the Guide harmless from and against any and all claims of loss, damage, liabilities, or expense whatsoever, either directly or indirectly, except any loss, damage, injury, or other claim arising from the above mentioned risks while participating in hunting and other associated activities on the property, whether or not the injury resulted whole or in part from any negligence by the Guide. I further agree that the Guide have no duty to warn me of any hazard or risk on the property including its roads and improvements. I am responsible to assess actual conditions and assume driving risks. I agree to remain with the Guide at all times and to obey all rules that have been made known to me verbally or in writing.

As further consideration of being allowed to hunt, or assist in the hunt as a guest, on the properties, the undersigned Hunter or guest hereby releases, waives, discharges and covenants not to file suit against the Guide for any and all losses, damages injuries or other claims or demands therefore, on account of injury to any person or property of the undersigned Hunter or guest, or resulting in the death of the undersigned Hunter or guest, directly, or indirectly any loss, damage, injury, or other claim arising from any negligence by Guide.
